Tennessee school vaccine exemptions

Religious exemption. A parent or guardian provides a signed written statement that immunization conflicts with their religious tenets and practices. The Tennessee Department of Health publishes an optional form, and schools may accept a letter. No notarization or provider signature is required.

Medical exemption. A licensed physician (or, in some cases, another authorized provider) documents the exemption on the Tennessee Certificate of Immunization, indicating which vaccines are medically contraindicated and whether the exemption is temporary or permanent.

What to ask a practice. Whether they will complete the Tennessee Certificate of Immunization reflecting a partial or delayed schedule, and whether they will document a medical exemption when one applies.

Where rules change. Requirements apply at childcare entry, kindergarten, and seventh grade. Confirm current forms with your school or the county health department.
